Surge in Cyclosporiasis Cases Reported Across Multiple States

Cyclosporiasis, a parasitic illness characterized by severe diarrhea, is experiencing a surge in cases across more than a dozen states. Michigan has become a significant hotspot, with the state’s Health and Human Services (HHS) department revealing over 1,500 cases and 44 hospitalizations as of July 9. Typically, Michigan records around 50 infections annually, so this surge is highly unusual. The source of the outbreak in Michigan remains unidentified.

The outbreak is spreading to other states, with Ohio reporting 177 cases, Illinois 141, New York 120, North Carolina 110, and Texas 48. These increasing numbers have attracted national attention as health authorities work to determine if these are isolated incidents or part of a broader trend. The Centers for Disease Control and Prevention (CDC) identified 145 domestically acquired cases across 17 states between May 1 and June 16; however, the actual scale may be larger due to delays in national surveillance compared to state-level reporting.

According to David Relman from Stanford Medicine, inconsistent reporting is affecting the picture of cyclosporiasis incidence. He noted the lack of information regarding clustering and common exposure sources. The cutbacks on foodborne illness monitoring by the Trump administration’s CDC have impacted surveillance capabilities.

How Unusual Is This Year’s Increase in Cases?

Keith Schneider from the University of Florida expressed concern over the spike in cases. Comprehensive historical data on cyclospora outbreaks is limited, but in 2022, Florida recorded 513 infections linked to contaminated lettuce. Previous peaks happened in 2018 and 2019, with 3,519 and 4,703 cases respectively.

Over 1,000 cases have been reported halfway through the current season from May to August, as per CDC typical patterns linked with fresh produce consumption. Jennifer Acuff from the University of Arkansas indicates the severity and rapid expansion of the outbreak is unusual.

What Foods Is Cyclospora Tied To?

Fresh produce contaminated by water or human waste remains the primary source of cyclospora infections. Lee-Ann Jaykus from NC State University outlined that vegetables, herbs, and certain fruits are commonly linked. Larger outbreaks often involve multi-component foods like salad mixes, complicating source identification.

Pinpointing the exact source is challenging because symptoms may appear a week after exposure, making it difficult for individuals to recall consumed foods. Unverified reports claim some Taco Bell restaurants halted serving specific ingredients such as lettuce; Taco Bell has yet to comment.

How Can You Protect Yourself From Cyclosporiasis?

Cyclosporiasis results from Cyclospora cayetanensis, contracted typically through contaminated food or water. Washing produce is a recommended preventive measure but does not eliminate the parasite entirely. Symptoms include diarrhea, cramps, bloating, nausea, fatigue, and loss of appetite, occurring roughly a week post-exposure. Although rarely fatal, the illness can persist and recur if not treated and poses higher risks for those with weakened immune systems.

Acuff advises those immunocompromised to avoid fresh produce until identifying the source. Relman emphasizes the importance of effective washing and farm preventive measures over mere reliance on hope for a safe food supply.
